Job description:

The Cardinal Ambrozic Houses of Providence is home to 288 residents of various backgrounds, perspectives and life experiences. With helping residents maintain a high quality of life our primary focus, we strive to recognize and respect the privacy and independence of all residents. Located conveniently across from Warden Subway station, on the Providence Healthcare campus, the Houses of Providence is also close to a beautiful greenspace that allows trail access for cyclists. We are a University of Toronto community affiliated teaching site.

The Department of Rehabilitative Care is currently seeking a dedicated, highly skilled long-term care physician to provide compassionate, resident centred medical care as a member of the inter-professional care team in the Houses of Providence.

Qualifications: 

  • Canadian College of Family Physicians (CCFP) certification
  • Care of the elderly designation an asset
  • Experience working in Long Term Care preferred
  • Must have an academic appointment at the University of Toronto as a Lecturer, Assistant,
  • Associate or Full Professor
  • Strong knowledge, skills and experience in palliative care including goals of care conversations
  • Experience in medical education, quality improvement and/or research an asset
  • Strong leadership and communication skills
  • Ability to work effectively as part of an interdisciplinary team

Role Responsibilities 

  • Most responsible physician for approximately seventy-two residents
  • Attendance at the Home bi-weekly in the provision of clinical care to assigned residents
  • Works effectively with members of the inter-disciplinary team including the Medical Director to ensure the highest level of care is provided to residents
  • Participation in department on-call schedule (approximately one half day per week of on-site coverage and 2nd on-call 3-4 weekends per year) - Provision of holistic, resident centred care in accordance with a resident’s values and wishes
  • Participation in Providence Healthcare and/or Unity Health Toronto committees including attendance at monthly Department of Rehabilitative Care meetings for medical staff
  • Teaching and supervision of medical learners
  • All physicians are expected to contribute to our ongoing efforts to improve the quality of the services we deliver. If not engaged in organizing quality improvement (QI) projects directly, all physicians are expected to contribute to QI by participating in QI projects whenever required
